Grant of Prosthetic Aid for fitting-up/repairing of Artificial Limbs

Description of the Scheme Under this scheme the costs of the
Specific prosthetic aid for fitting up/repairing of Artificial Limbs will be provided by the Social Welfare department.
The application forms are available with the District Social Welfare Officer (DSWO). The application shall be accompanied by:
1. A certificate on the prescribed form by the Surgeon/Orthopedic/ENT specialist, countersigned by the Superintendents of the Hospitals/District Medical Officers indicating the type of disability and appropriate prosthetic aid required by the applicant.
2. A certificate on the prescribed form from Revenue Officer not below the rank of Tehsildar or an affidavit attested by a Gazetted Officer/1st Class Magistrate or a certificate from a Gazetted Officer of Central/State Government or a Member of the Parliament/State Legislature indicating the monthly income of the family from all sources.
 
Eligibility

Disabled persons in need of prosthetic aid whose family income is less than Rs. 400/- per month.
On receipt of application duly completed by the applicant with regard to the disability and type of prosthetic aid required .DSWO shall consolidate the list and forward to the District Level Sanctioning Committee, comprising of Chief Medical officer as member, DSWO as member Secretary and Deputy Commissioner as Chairman. The Committee authorizes the District Social Welfare Officer to purchase these aids/fitments from the approved agencies for onward disbursement among the sanctioned beneficiaries.
